This certified report shows whether there is on file any currently effective UCC financing statements filed on or after January 1, 1995 for a specified name.

Certified search reports can be obtained by completing the UCC-11 Form and submitting it to the GSCCCA. The form is available online as an Adobe PDF document. Please refer to the UCC-11 Filing Checklist to ensure your request is processed correctly. The UCC-11 Form must be typed and submitted in duplicate.
