UCC Financing Statement (UCC-1) Filing Checklist
1. Use only the Authority-approved forms. If using the online form (UCC Financing
Statement), you may electronically enter information into the data fields. The State
of Georgia only accepts the forms with a revision date of 07/01/2023, or later.
2. A majority of Superior Court Clerks now mandate electronic filing of UCCs. Please go
to Mandatory eFiling Counties for a list of counties requiring eFiling of UCCs.

UCC-11 Information Request Checklist
UCC-11 Information Request results return ONLY information related to
UCC filings. No information can be returned for any other type of document.

enter information into the data fields.

The fee is $15 per, NOT ENTER NAMES IN BOTH 1A AND 1B. Use additional forms for more
than one name.
